This program is for children and youth from ages birth to 21 who have a verified chronic medical condition. Coordinators work together with families, their health care providers, and other community agencies to help obtain access to needed health care and related services. This program also supports the family in obtaining appropriate community social supports and services. Health Care Coordinators work with families and young adults towards identifying and supporting needs and goals for a more independent community and family life.
