Transition Coordinator
Janice was formerly a Family Support Coordinator for the Partners in Health Program a position she held for over 20 years. She has spent many years a Licensed Practical Nurse in a variety of fields that included hospital, Visiting Nurse and Clinic ( NeuroMotor ) settings. She has her Bachelor’s Degree in Behavioral Science.
Janice has a passion for working with teens with chronic health conditions transition to adult health care. She has two adult sons with chronic health conditions that as a team they personally experienced health care transition.
