Partners In Health – Region I Berlin/Lancaster

The New Hampshire Partners in Health Program helps families of children with a chronic health condition that significantly impacts daily life. Partners in Health’s role is to advocate, access resources, navigate systems and build capacity to manage the chronic health condition of their child. In Region I Berlin/Lancaster the PIH Coordinator works out of Waypoint. They cover the towns of: Berlin, Carroll, Clarksville, Colebrook, Columbia, Dalton, Dixville, Dummer, Erroll, Gorham, Jefferson, Lancaster, Milan, Northumberland, Pittsburg, Randolph, Shelburne, Stark, Stewartstown, Stratford, Wentworth and Whitefield.
